Newly Registered Patients

As long as you live within the surgery boundary you can register. Talk to reception, they will give you a form to complete (GMS1).

A Health Visitor will contact parents of under fives. You will be registered with the surgery but you have the right to express a preference of a GP if you wish, please ask reception. If you are over 75 you will be given a named GP who will be responsible for your overall care but this does not stop you from seeing any GP.

Disabled Patients

The Surgery is fully equipped for disabled access, including toilet facilities.

Language Line

We do have access to language line if you have difficulty understanding what the clinicians say to you. Please ask at reception when you make an appointment. And you will be given a double appointment.

Temporary Residents

We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.
